Kunio, a tough guy from Nekketsu High School, stands up for his bullied friend Hiroshi against the numerous gangs that are tormenting his life. Each stage begins with one of the gang's members beating up Hiroshi, followed by Kunio chasing after the attackers.
The game is comprised of four stages, each with their own unique set of enemy characters. Once you have defeated a certain amount of enemies, the gang leader will come out of the background and help the remaining members beat you up. Once you have defeated the leader, you will see a scene of another gang attacking Hiroshi and you will proceed to the next stage. After all four leaders have been defeated, the game restarts but at a higher difficulty level.
